If you take a simple stroll down the foot product isle at your local drug store you will find an array of foot products and inserts. You will find inserts for cushion, for comfort, for support and for control. Many questions may cross your mind. What are the differences? Is the $15 insert better than the $7 insert? Are these orthotics? Will they help my foot pain? You may take the time to study the package details closely, but in most instances it doesn’t really matter which pair you grab. Most of the inserts at the local drug store are essentially the same.

An insert slips into the shoe and will give you some comfort, cushion and a little support. In some cases, inserts purchased at the drugstore will help alleviate some arch or heel pain and give relief to sore, tired feet. But, an insert is not an orthotic. Orthotics are devices which fit into the shoe and aid in foot function. The most important difference is an orthotic controls abnormal motion and corrects foot function, while an insert will only provide cushion and comfort.

A prefabricated orthotic is a device that is pre-made and designed to fit the most common foot types. Typically, a prefabricated orthotic needs to be purchased at a sports store and they are more expensive than inserts, costing $25-$30. The prefab orthotic is more rigid than an insert and is designed not only to support the arch, but also to help control motion, which will decrease stress on the tendons and ligaments in the arch. Many individuals will do very well with a prefabricated orthotic. Prefab orthotics generally last for one or two years before replacement is necessary.

A functional custom made orthotic is a device that is even more rigid than a prefab orthotic. The custom made orthotic must be rigid to help control the motion in the foot and add support. If the orthotic is soft, the weight of the body would collapse the device and it would no longer function. A custom made orthotic is not only custom molded to your foot and arch, corrective wedges and covers are built into the device to help with motion control and to improve the mechanics of the foot. Individuals with flatfeet, tendonitis, plantar fasciitis, certain foot deformities, knee, hip and back problems may benefit from custom made functional orthotics. The orthotic should make standing, walking, jogging or running more comfortable. Custom made functional orthotics will generally last 7-12 years before needing to be replaced, and can cost between $300-$600.

Functional orthotics are a successful treatment for many problems affecting the lower extremity. In a recent article in the Journal of the American Podiatric Medical Association, 75% of patients surveyed had good to excellent results using functional orthotics to treat heel pain, arch pain, arthritis, bunions, pain from flatfeet or high arches, knee, hip and back pain. A recent article in USA Weekend magazine stated the opposite. The article referenced a study in Foot & Ankle International in which researchers followed 874 healthy, young Israeli army recruits without foot problems. The recruits were given soft custom, semi-rigid custom, soft prefab or semi-rigid prefab orthotics to wear in their boots during basic training. The researchers found no difference in the development of foot problems between the custom made orthotic group versus the prefab orthotic group. The conclusion from this study is that custom made orthotics will not help prevent foot problems from developing. What is important to understand is this study did not evaluate individuals with foot pain or foot problems. The population evaluated was a group of young, healthy, active individuals without any existing foot problems. Therefore, we cannot conclude from this study that prefab orthotics treat foot conditions better than custom-made orthotics.

Individuals with plantar fasciitis (heel and arch pain) who also have flatfeet usually respond best to custom-made orthotics. Certain types of tendonitis respond well to orthotics and other types require orthotics. Posterior tibial tendonitis is the tearing and inflammation of the tendon that helps to hold up the arch. When this tendon is overstressed by arch collapse it cannot heal. Custom made orthotics are essential in the treatment of this condition.

Individuals with high arches may also require orthotics, but they do not respond as well. Custom made orthotics can help slow the progression of bunions and hammertoes, but they will not prevent this process. Orthotics may help with some pain at a bunion, but they will not “cure” the bunion. When the motion in the foot is contributing to the problem, orthotics are generally recommended. If the foot is stable and does not require support, the bunion, hammertoe, neuroma, tendonitis or even plantar fasciitis may not require custom made orthotics for treatment. These individuals may do well with a pre-fabricated orthotic. If you do not have foot pain or an existing foot condition, a custom made orthotic is not necessary for prevention of foot problems.

A custom made accommodative orthotic is soft and designed to reduce pressure and prevent excess friction. Diabetics can develop numbness and loss of circulation in the feet. This numbness and circulation loss puts them at risk for developing open sores on the feet called ulcerations. To help prevent excess rub and friction in certain areas on the feet, accommodative orthotics are recommended. Diabetics who do not have numbness or circulation loss (as diagnosed by their doctor) do not need to have accommodative orthotics, unless the foot has a deformity, like a bunion or hammertoe, then accommodative orthotics are necessary.

Both types of custom-made orthotics are formed by taking a mold of the foot. A functional orthotic mold is fabricated by wrapping the foot in plaster. The foot is held in a corrected position while the plaster is setting. To obtain an accommodative orthotic mold, the patient steps into a foam box. An impression of the foot in a standing position is then created. The molds are sent to a lab and scanned into a computer. A reverse image is produced and the computer generates an image of the foot in neutral position. The computer adjusts the image based on the corrections recommended. A model of the foot is then cut out, in some cases out of wood. The orthotic material is pressed over the foot model and the orthotic is created. The most common material for a semi-rigid custom made orthotic is polypropylene, but other materials, such as graphite are used. A foam-like material is typically used for custom-made accommodative orthotics.

The bottom line is that if you have foot pain, a foot deformity or condition, let your podiatrist evaluate you and decide if orthotics should be part of the treatment. If your insurance will not cover the orthotics and you cannot afford custom made orthotics, consider trying prefabricated orthotics. If you are looking to help prevent foot problems or looking for some extra support, a prefab orthotics is a good option for you, especially if you are active in sports. If you are just looking for a little extra cushion and comfort, an insert at your local drug store will be your best bet.

Cetyl Myristoleate is a cure for arthritis. This is not only a bogus claim it is a lie. Not only is it a lie it is illegal to make that claim. If you are at a web site that makes this claim, leave, this person is not the kind of person you want to do business with. They need to be reported to the Federal Trade Commission.

All you need is one 15 or 20 day protocol. There are several companies that make this type of claim. You will notice that the companies that make this claim are among the most expensive. I believe that they use this to justify their high prices. Who would pay this price on a regular bases. While the double blind studies show that many people start finding relief in this amount of time, they were all short term studies and did not evaluate how long the results lasted. Every doctor I have talked to has disagreed with this statement. The people who find relief from Cetyl Myristoleate usually start seeing good results by the two week mark. But they continue to improve for the next two to four weeks. Almost everyone who discontinues use finds that with in a couple of weeks their symptoms begin to return. But they also find that once they max out their benefit they need much less to maintain that level of relief.

Cetyl Myristoleate helps 97% if the people who use it. I have seen this claim several times. It is simply not true. The research does not back it up and neither do those doctors who use it in practice. The percentage is closer to 70%. Of course the percentage changes depending on the type of arthritis you have. With nearly 100 types of arthritis nothing is going to work well on every type.

Cetyl Myristoleate is an anti aging agent. This is a powerful marketing tool. Every one wants to look and stay young. There is no research to back up this claim. Some of the doctors I have talked to believe it base on their observations. I have over a dozen family members and friends who take CM faithfully and none of us look younger. Yet almost all of us feel younger because we can now do things again that we had to give up because of our arthritis. If this is what they are talking about I concur. But if they are claiming it will make you younger or keep you from aging I think they are stretching it.

It is necessary to take digestive enzymes with Cetyl Myristoleate. This is a hard one. I personally do not think everyone does. Some people have a hard time digesting fats. If taking CM causes you stomach upset then you need to take a digestive enzyme. Make sure it has lipase because it is the enzyme that digests fat.

Cetyl Myristoleate will help you grow new cartilage. This is another unfounded claim. There is nothing in CM to help you grow cartilage. Once the inflammation is down then your body may find it easier to replace the cartilage damaged by the inflammation.

Will Cetyl Myristoleate help me with my arthritis? I would like to end the article with the question most ask of me. The answer is I do not know. All I can say is the research and my experience and the experience of the doctors I have talked to says that there is a 60% to 70% chance. Be reasonable about your expectations. CM is not going to repair bone damage, remove calcium deposits or repair other types of damage created by your arthritis. If you decide to try it do not pay too much. There is no need to pay $50, $80, $100 dollars or more. There are several good products in the $20 to $50 range. If the first bottle does not work for you do not waste your money on a second.

When you ask what arthritis is, professinals will tell you it’s inflammation of one or more joints. But you know it better as pain, swelling, stiffness, deformity, and/or a diminished range of motion of those joints! Some people try unproven remedies which also delay proper diagnosis and treatment.

Since arthritis may evolve gradually, people often ignore its early warning symptoms or signs. These include persistent pain, tenderness, or swelling in one or more joints?symptoms that should not be dismissed as signs of age.

Other warning symptoms are joint pain and stiffness?especially when they appear in the morning.

But, these types of arthritis differ in terms of?age of patients who are affected?the joints involved?the pattern of stiffness?and the potential for disability.

Close to 16 million Americans have osteoarthritis ? the most common type of arthritis. Although osteoarthritis can occur at any age, it most often begins in people in their 50’s and 60’s.

Osteoarthritis or degenerative disc disease is a disorder of cartilage ? the gristle that covers the ends of long bones. Cartilage is made of cell called chondrocytes which sit inside a framework made up of collagen and proteoglyens. Under normal conditions, chondrocytes make collagen and proteoglycens ? in other works ? they make the framework they sit inside. With osteoarthritis, chondrocytes behave abnormally and begin to make destructive enzymes such as collagenasese, stromelysin and others. These enzymes degrade cartilage?these enzymes also attract inflammatory cells which secrete substances called cytokines which cause further inflammation and damage to cartilage, underlying bone, and the joint lining.

This process results in progressive pain, stiffness, and loss of function.

Joint pain and stiffness are the most noticeable symptoms of osteoarthritis. Morning stiffness is usually brief lasting less than 15 minutes. Osteoarthritis usually affects weight bearing areas particularly the neck, low back, hips and knees.

It may also affect the fingers and hands and bony knobs may appear at the finger joints. The base of the thumb may also be affected. The typical pattern of osteoarthritis in the hands involves the distal and proximal interphalangeal (DIP and PIP) joints of the fingers, and the carpometacarpal (CMC) joint of the thumb.

Osteoarthritis is considered to be a degenerative joint disease. Along with inflammation, there is wear and tear on the inside of the joint.

This causes damage to the cartilage (the substance that forms the surface of the joints and works as a shock absorber). As the cartilage wears thin, the underlying bone is damaged. This process results in progressive pain, stiffness, and loss of function.

Osteoarthritis does not need to be disabling and with the proper medical care can be managed easily.

Rheumatoid arthritis is the other most common type of arthritis. It is more common in women and affects 7 million Americans?or one out of every five arthritis patients. It may affect any age group, although onset is most common in middle age.

Rheumatoid arthritis is usually characterized by heat, swelling, and pain in multiple joints in both the right and left sides of the body, including the hands, wrists, elbows, hips, knees, ankles, and feet. Spinal involvement also occurs on occasion.

The typical pattern of rheumatoid arthritis in the hands involves the proximal interphalangeal (PIP) joints, the metacarpal phalangeal (MCP) joints, the wrists, and the elbows.

Unlike osteoarthritis, rheumatoid arthritis can affect the entire body. People with this disease may feel sick all over?tire easily?lose their appetite?and lose weight.

In rheumatoid arthritis, the tissue that surrounds and nourishes the joints is attacked by the body’s immune system. The body mistakenly perceives its own tissue as foreign, and it reacts by sending special white blood cells and toxic chemicals called cytokines to destroy the foreign material. (The cytokine abnormalities that cause the damage in rheumatoid arthritis are different from the abnormalities seen in osteoarthritis.) This process of white cell migration and cytokine release damages the joint.

Although we do not know the cause of rheumatoid arthritis, researchers are investigating several possibilities.

Another interesting point about rheumatoid arthritis is that this disease can affect the internal organs including the lungs, skin, blood vessels, spleen, heart, and muscles.

If rheumatoid arthritis is not well controlled it can damage the joints irreversibly and cause serious disability.

To diagnose rheumatoid arthritis, the rheumatologist establishes the presence of joint pain and inflammation lasting at least six weeks and then looks for signs of the course of the disease that are characteristic for rheumatoid arthritis.

There are also blood tests that aid in the diagnosis of rheumatoid arthritis.

Patients with rheumatoid arthritis have a series of flare-ups followed by a period where there are mild or no symptoms. Usually, the pain and disability of rheumatoid arthritis progresses gradually.

Morning stiffness generally lasts longer than half an hour and may last several hours depending on the severity of the condition.

Most forms of arthritis persist for the patient’s lifetime. Medication cannot usually reverse the bone and soft tissue damage caused by arthritis.

However, new methods of measuring inflammation and its response to medication and other treatments offer valuable information to physicians?and can help to evaluate the arthritis sufferer’s discomfort.

Magnetic resonance imaging is one such technique. This method using the effects that strong magnets have on water molecules to provide exquisite images of the interior of the body. MRI has been used to diagnose and also assess the degree of damage within joints of patients suffering from arthritis. It is also helpful for evaluating the effect of new drugs.

Although there is no cure for arthritis, proper treatment can help tremendously. The goal of arthritis treatment is to relieve the pain and stiffness due to the progressive destruction caused by inflammation, and to maintain or increase freedom of movement.

Among the advancements that have taken place in the medical treatment of arthritis are various disease-modifying medications that not only relieve symptoms but also help slow down the progression of disease.

Other advances include various cartilage sparing drugs, cartilage growing drugs, and also biologic remedies. These drugs act by blocking the destructive effects of enzymes such as metalloproteases in osteoarthritis and cytokines in rheumatoid arthritis. By targeting specific processes, relief of symptoms and healing of damage can take place with presumably fewer side effects.

What can you do if you think you have arthritis?

First, you can consult your doctor. This is important because medical issues are complicated and your doctor, who understands your health needs, can prescribe the best treatment for you.

The type of doctor who can best evaluate arthritis is called a rheumatologist. These are physicians who have completed four years of medical school, three years of internal medicine residency, and three years of rheumatology fellowship.

While arthritis can be a serious disease that can progress and cause disability, science has come up with some new answers for arthritis sufferers. It is now up to the arthritis sufferer to recognize early warning signs and symptoms and to see a rheumatologist.
